Whatever the type or progression of glaucoma, we can likely treat it

  • We cannot reverse the damage caused by glaucoma. But treatment and regular checkups can help slow or prevent vision loss, especially if you catch the disease in its early stages.
  • We treat glaucoma by lowering your eye pressure (intraocular pressure). Depending on your situation and the type of glaucoma you have, your options may include prescription eye drops, oral medications, laser treatment, surgery or a combination of any of these.
Treating open-angle glaucoma

  • Glaucoma treatment often starts with prescription eye drops. These can help decrease eye pressure by improving how fluid drains from your eye or by reducing the amount of fluid your eye produces.

Oral medications

  • If eye drops alone don’t bring your eye pressure down to the desired level, we may also prescribe an oral medication.
Laser trabeculoplasty and surgery
  • Other treatment options include laser therapy and various surgical procedures.
  • The following techniques aim to improve the drainage of fluid within the eye, thereby lowering pressure:
  • Laser therapy. Laser trabeculoplasty is an option if you have open-angle glaucoma. Here we use a small laser beam to open clogged channels in the trabecular meshwork. It may take a few weeks before the full effect of this procedure becomes apparent.
  • Filtering surgery. With a trabeculectomy surgical procedure, we create an opening in the white of the eye (sclera) and remove part of the trabecular meshwork.
  • Minimally invasive glaucoma surgery (MIGS). We may suggest a MIGS procedure to lower your eye pressure. These procedures generally require less immediate postoperative care and involve less risk than trabeculectomy or drainage tube insert. There are a number of MIGS techniques available. We will discuss which procedure may be right for you.
  • After your procedure, you’ll need to come in for follow-up exams. And you may eventually need to undergo additional procedures if your eye pressure begins to rise or other changes occur in your eye.
Treating angle-closure glaucoma
  • Acute angle-closure glaucoma is a medical emergency. If you’re diagnosed with this condition, you’ll need urgent treatment to reduce the pressure in your eye. This generally will require both medication and laser or other surgical procedures.
  • You may have a laser peripheral iridotomy procedure in which the doctor creates a small opening in your iris using a laser. This allows fluid (aqueous humour) to flow through it, relieving eye pressure.
Eye drops and medications

Advantages

Medications have several advantages, including easy tolerability in most cases, low intervention, and good effectiveness.

Disadvantages

However, downsides include side effects such as eye irritation and redness or allergic reactions, the need to remember to take the medications every day, and ongoing cost depending on your prescription coverage.

Laser trabeculoplasty

Advantages

  • Laser trabeculoplasty is effective and relieves you from having to take drops (if your eye pressure is fairly easy to control) and has few side effects.

Disadvantages

  • The disadvantages of laser treatment include the fact that it is a procedure (and there are risks with any procedure), it is not a cure, and the effect tends to wear off over time.
  • For some patients, the answer is clear: they do not want to undergo a procedure, and thus eye drops are preferred. However, other patients prefer laser treatment because they do not want to have to remember to install eye drops or experience their side effects.
Surgery

Advantages

  • Surgery has very high success rates in substantially slowing the progression of glaucoma.

Disadvantages

The disadvantage of surgery is the potential complications and side effects. Depending on the type of surgery performed, complications can include:

  • Intraocular pressure too low or too high.
  • Excessive intraocular inflammation.
  • Bleeding within the eye.
  • Blurred vision after surgery.
  • Sight-threatening infection.
Eye drops and medications risks
  • The risks involved in eye drops and medications include side effects such as eye irritation, redness or allergic reactions, and the need to remember to take the medications every day.
Laser trabeculoplasty risks
  • Post-operative inflammation is common but generally mild and treated with eye drops or an oral non-steroidal anti-inflammatory drug. There is an approximately 5% incidence of increased eye pressure after laser, which we can manage with glaucoma medications and usually goes away after 24 hours.
Surgery risks
  • When it comes to surgical intervention for advanced glaucoma cases, the benefits typically outweigh the risks.
  • The following are (rare) glaucoma surgery risks or side effects:
    • Vision loss
    • Bleeding in the eye
    • Infection
    • Low eye pressure (or hypotony)
    • Scarring
    • Cataract formation

  • Treating glaucoma is a team effort between you and your doctor. First, your ophthalmologist will prescribe your glaucoma treatment. After that, it is up to you to follow your doctor’s instructions and use your eye drops.
Eye drops and medications steps
  • Once you are taking medications for glaucoma, we will need to see you regularly. You can expect to visit us about every 3–6 months.
  • However, this can vary depending on your treatment needs.
Laser trabeculoplasty steps
  • Step 1: We will assess your eye and check the pressure.
  • Step 2: We will apply some more drops into your eye, including a drop to protect the eye from any spikes in pressure and an anaesthetic drop to numb the eye. We usually put in a drop to make the pupil smaller.
  • Step 3: You will then sit at a machine like the one used to examine your eyes at the eye clinic, but with a special laser attached to it. We will put a special contact lens on your eye before applying the laser beam. This lens allows us to view your eye clearly so they can apply the laser to the drainage channels. The treatment is painless because of the anaesthetic drops used to numb your eye.
Surgery steps
  • Surgery involves making a cut in the eye to reduce intraocular pressure. The type of surgery your doctor recommends will depend on the type and severity of your glaucoma and the general health of your eye.
